The pilot of a Midwest Express Airlines DC-9 should have been able to keep his twin-engine jetliner aloft despite engine failure and to avoid the fatal 1985 crash in Milwaukee, a federal investigation concluded. The National Transportation Safety Board said the “improper use of flight controls” in response to the engine failure seconds after takeoff was to blame for the Sept. 6, 1985, accident in which all 31 people aboard the jetliner were killed.
