Dodger Problems Point to Lasorda

The Dodgers two seasons running now have had acute bullpen problems. This season also the starting rotation is “Hold your breath.” My question is this: When does management/ownership begin to question the job a pitching coach is doing? Responsibility has to fall upon someone’s shoulders. Also, how come Tommy Lasorda no longer even goes to the pitcher’s mound? I would think some situations would call for his presence out there.
